As a college or university student with a disability, you can access FormEdOn's specialized academic tutoring through the Canada Student Grant for Services and Equipment.

This guide walks you through the process of accessing FormEdOn's tutoring and executive function support services step-by-step. Whether you're applying for the first time or optimizing existing funding, we've simplified the journey to help you get the academic support you need. Your path to success is unique, and we're here to help you navigate it.


Pre-Application Phase

  1. Verify your eligibility requirements

    • Student Status Verification:

      • Be enrolled in a designated post-secondary institution in Canada

      • Registration can be full-time or part-time

      • Must be in a program that leads to a degree, diploma, or certificate

    • Disability Documentation Requirements:

      • Documentation must be from a qualified medical assessor (physician, psychiatrist, psychologist, etc.)

      • Documentation should be recent (typically within the last 5 years for permanent disabilities)

      • Must clearly state the nature of your disability (learning disability, ADHD, physical disability, mental health condition, etc.)

      • Documentation should explain how your disability specifically impacts academic functioning

      • For learning disabilities, a full psychoeducational assessment is usually required

    • Financial Eligibility:

      • Must demonstrate financial need through the standard Canada Student Loans assessment

      • Financial need threshold is lower for disability-related grants than standard loans

      • Students who are not eligible for loans may still qualify for the Canada Student Grant for Services and Equipment for Students with Permanent Disabilities


    2.Evidence of Academic Need

    • Document how your specific courses require support services

Application Process

Maintaining Services

  1. Document your progress and benefits

    • Keep records of how the tutoring is helping your academic performance

    • Collect evidence (improved grades, completed assignments) for future applications

    • Complete any required feedback forms

  2. Renew for subsequent terms

    • Update your documentation if your academic or medical circumstances change

    • Submit new Appendix 8 forms for each academic year or as required by your institution
